All competitors who enter any Transplant Sport Satellite event must complete the Transplant Sport Medical Wavier Form.
The Wavier form can be found on the link below, once completed please email it to office@transplantsport.org.uk
If you are less that 6 months post-transplant or on Dialysis you will need to complete the Transplant Sport Medical Form, please email office@transplantsport.org.uk and we will email the form to you.
